A Case of Unusual Acute Monocytic Leukemia of Shilling Type






Abstract
A case of monocytic leukemia of Schilling type is presented. A 52 year old Korean male was admitted to E.N.T. Department of Woo-Sok Hospital of Korea University because of frequent nasal bleeding. During three weeks before entry he was treated with moxibustion for diffuse swelling of cervical lymphnodes. On the first day of admission peripheral blood showed marked leukocytosis (108. 000/mm¢¥) and severe anemia (Hb. 5.6gm/di, Ht. 18%). During admission the leukocyte count was 108.000/mm3 with markedly increase in blast form (40%) and promonocyte (39%). Platelet count revealed thrombocytopenia (70, 000/mm3). Bone marrow blood smear showed marked increase in blast form (44%), promonocyte (37.4%) mop ocyte (11.2%), and marked decrease in myeloid and erythroid series.
Blood smears reveal unusual characteristics of cell morphology to match Schilling type but markedly decrease in myeloid and lymphocytic series with lymphnode swelling is highly suggestive that of Schilling type acute monocytic leukemia.
